Overview
What filler actually does
With time, the face quietly loses volume: cheeks flatten, temples hollow, lips thin and shadows deepen around the mouth and under the eyes. Dermal filler replaces what has been lost, or adds a little definition where you have always wanted it, using hyaluronic acid, the same molecule your skin makes on its own.
Filler is planned across the whole face rather than one feature at a time. Lifting the cheek, for instance, often softens the smile lines and the under-eye at the same time, with less product than treating each spot on its own.
Because hyaluronic acid can be dissolved with Hylenex®, a filler decision is never a permanent one.

A fine line of filler along the edge of the lip to sharpen its outline and bring back a cupid's bow that has softened with time.

The fine vertical lines above the upper lip. Smoothed with a soft, low-volume filler so lipstick stays exactly where it is put.

Under-eyesTear troughs

Your filler appointment, step by step

Consultation
Dr. Alex, DNP, asks what you notice in the mirror, reviews your health history, and looks at your face as a whole, in proportion. You hear what she recommends, how many syringes, why, and the cost at your office before anything is opened.

Treatment
A numbing cream is applied, and most fillers also contain lidocaine. Filler is placed with a fine needle or cannula in small amounts, checking the shape as she goes. A first appointment runs about an hour, consultation included.

Aftercare
Some swelling is normal, especially in the lips, and settles over a few days; a small bruise is possible. Skip hard exercise and alcohol for the rest of the day, and avoid pressing on the treated areas.

Results & follow-up
The change shows right away and settles as swelling fades, at about two weeks, when a follow-up lets Dr. Alex, DNP, refine if needed. Results fade gradually over six to eighteen months, depending on the area.

Choose your officeHow much does filler cost?
Filler is priced per syringe, and pricing differs from office to office. Most first lip treatments use one syringe or less — many people are happiest starting with half. Dr. Alex, DNP, gives you the exact figure at the consultation, after looking at your lips in proportion to the rest of your face and hearing what you are hoping for. Cheeks, jawline and chin usually call for more product than lips, and you will know the number before anything is opened.
What are dermal fillers made of?
The fillers used here are made of hyaluronic acid, a sugar molecule your skin already produces to hold water and keep itself plump. In a filler it is formed into a smooth gel that adds volume and structure where it is placed, and is broken down naturally by the body over time. Sculptra, a collagen stimulator, is the exception — it works by prompting your skin to build its own collagen.
How long do fillers last?
It depends on the area and the product. Lips, which move constantly, typically hold for six to twelve months; cheeks, chin and jawline use firmer fillers that can last twelve to eighteen months or longer. Results fade gradually rather than all at once, so there is never a moment where it suddenly looks gone.
Will my lips look overdone?
Not here. Dr. Alex, DNP, builds lips in small amounts and in proportion to the rest of your face, and would rather see you back in a few weeks for a little more than send you home with too much. The lips people admire are the ones no one can quite put their finger on — that is what we are aiming for.
Does filler hurt, and will I bruise or swell?
Most modern fillers contain lidocaine, and a numbing cream is applied before treatment, so most people describe pressure or a pinch rather than pain. Some swelling is normal, especially in the lips, and settles within a few days; a small bruise is possible and easily covered. Skip alcohol, fish oil, ibuprofen and aspirin for a few days beforehand if you can, and plan filler at least two weeks ahead of a wedding or a photo you care about.
Can filler be dissolved?
Yes. Hyaluronic acid filler can be dissolved with an enzyme called hyaluronidase, usually within a day or two, which is one of the reasons Dr. Alex, DNP, works in hyaluronic acid. It is rarely needed — but it means a filler decision is never a permanent one, and older filler placed elsewhere that has migrated or looks heavy can be cleared before starting fresh.
Do you treat under-eyes and temples?
Yes, both — as part of a plan for the whole face rather than one spot in isolation. Temples that have hollowed narrow the upper face and pull the tail of the brow down; refilled subtly, they restore a soft, rested contour. Under-eyes are the most delicate area we treat, and the one where candidacy matters most: filler suits a true hollow between the lower lid and the cheek, not puffiness, heavy bags or very thin skin, and lifting the cheek first often does more for the under-eye than treating it directly. Dr. Alex, DNP, will tell you honestly whether you are a candidate.
How should I prepare, and how long is the visit?
Arrive with clean skin if you can, and avoid alcohol and blood-thinning supplements for a few days beforehand to keep bruising to a minimum. A first appointment runs about an hour: the consultation, numbing, the treatment itself, and a few minutes to look together before you leave. Every office is by appointment only, so book ahead, and you will have Dr. Alex, DNP's full attention for the whole of it.
